Please complete the intake below or email us at intake@bloomandgrowtherapy.com.  The treatment plan development process is uniquely tailored to each client.  A prescription from your physician is required for medically based Occupational and Speech-Language Therapy.

For out of network insurances-

Upon collaboration with the family, Bloom and Grow will complete a letter that outlines the plan of care, the medical need, as well as state the needed treatment frequency. The family will send this letter with their physician’s script, and their accounting statement of the initial evaluation to their insurance company. The insurance company will inform them of their reimbursement rate.

We do provide a monthly statement of reimbursable treatment codes for each visit to assist in reimbursement.
